Vitreo-Retinal Medical Group, Inc. d/b/a Retinal Consultants Medical Group

The covered entity (CE), Vitreo-Retinal Medical Group dba Retinal Consultants Medical Group, reported that it was the victim of a ransomware attack affecting the protected health information (PHI) of approximately 11,603 individuals. The PHI involved included names, Social Security numbers, dates of birth, financial and claims information, diagnoses, and health insurance and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. The CE also provided affected individuals with a dedicated number for questions or concerns. In response to the breach, the CE implemented additional administrative, technical, and security safeguards. OCR provided technical assistance to the CE.
