OCR description
The covered entity (CE), St. Anne’s Family Services, reported that it was the victim of a ransomware attack that affected the protected health information (PHI) of 661 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and health insurance and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional technical safeguards and retrained its staff.
