OCR description
The covered entity (CE), Ophthalmology Associates reported that it was a victim of a hacking incident that affected the protected health information (PHI) of 76,000 individuals. The PHI involved included names, addresses, dates of birth, and Social Security numbers. The CE notified HHS, the affected individuals, the media, and provided substitute notice. To mitigate harm, the CE implemented additional administrative and technical safeguards.
