OCR description
The covered entity (CE), Tom Wood Health Care Plan, reported that it experienced a cybersecurity incident that compromised the protected health information (PHI) of 2,235 individuals. The PHI involved included names, Social Security numbers, dates of birth, and benefits information. The CE notified HHS, affected individuals, and the media. The CE implemented additional administrative, technical, and security safeguards. OCR provided technical assistance.
