OCR description
The covered entity (CE), The Kroger Company, reported that a journal containing the protected health information (PHI) of up to 2,686 individuals was stolen. The PHI involved included names, medical record numbers, and medications prescribed. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE sanctioned the responsible employee and retrained its staff.
