OCR description
The covered entity (CE), Magnet Solutions, reported that an employee mailed the protected health information of 1,553 individuals to the wrong recipients. The PHI involved included names and treatment information. The CE notified HHS, the affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
