OCR description
The covered entity (CE), Anthem, reported that its business associate (BA) experienced a cyber-attack that compromised the protected health information of 6,118 individuals. The PHI involved included names, addresses, dates of birth, email addresses, telephone numbers and treatment information. The CE notified HHS, affected individuals, and the media of the breach. In response to the breach, the CE and BA implemented additional administrative and technical safeguards.
