OCR description
The covered entity (CE), United Health Centers of The San Joaquin Valley. reported that it experienced a cyber-attack affecting the protected health information (PHI) of 112,031 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, diagnoses, and medications. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional technical safeguards, revised its policies and procedures, and provided affected individuals with complimentary credit monitoring services.
