OCR description
The covered entity (CE), Onondaga County Chapter NYSARC, Inc., reported that it was the victim of a ransomware attack affecting the protected health information (PHI) of 5,425 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, diagnoses, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In its mitigation efforts, the CE provided complimentary credit monitoring services and implemented additional technical safeguards.
