OCR description
The covered entity (CE), Washington County School District, reported that its business associate (BA) experienced a malware attack that compromised the electronic protected health information (ePHI) of 659 individuals. The ePHI involved included names, Social Security numbers, drivers’ license numbers, addresses, dates of birth, diagnoses, financial and health insurance information, and other treatment information. The BA notified HHS, affected individuals, and the media. OCR provided technical assistance to the CE regarding the HIPAA Breach Notification Rule.
