OCR description
The covered entity (CE), One Community Health, reported that it experienced a cybersecurity attack that compromised the protected health information (PHI) of 39,865 individuals. The PHI involved included names, dates of birth, drivers’ license and Social Security numbers, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative and technical safeguards.
