OCR description
The covered entity (CE), Quantum Imaging and Therapeutics Associates, Inc., reported that it was the subject of a cybersecurity attack that affected the protected health information (PHI) of 394,506 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and other treatment information. The CE notified HHS and affected individuals. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect its PHI.
