OCR description
The covered entity (CE), Neuro Rehab Associates dba Northeast Rehabilitation Hospital Network, reported that it experienced a cyber-attack that affected the protected health information (PHI) of 190,220 individuals. The PHI involved included diagnoses and conditions. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE provided complimentary credit monitoring services and implemented additional administrative, technical, and security safeguards to better protect its PHI.
