OCR description
The covered entity (CE), Amistad Family Services, reported that a USB drive was stolen from its facility. This breach compromised the protected health information (PHI) of 2,915 individuals. The PHI involved included names, addresses, dates of birth, and diagnoses and conditions. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and physical security safeguards. The CE also retrained its workforce.
