OCR description
Roy Varughese, M.D., the covered entity (CE), reported that an employee was the victim of an email phishing scheme that compromised the protected health information of 2,916 individuals. The CE subsequently provided documentation notifying OCR that it was no longer a business entity. OCR then determined that it has no jurisdiction to investigate further.
