OCR description
The covered entity (CE), Virginia Department of Behavioral Health and Developmental Services, reported that the protected health information (PHI) of 4,037 individuals was made public on its funding portal. The PHI involved included names, mailing addresses, e-mail addresses, Social Security numbers, dates of birth, and phone numbers. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE immediately removed the portal from service and offered the affected individuals complimentary credit monitoring services.
