OCR description
The covered entity (CE), Mertz Manufacturing Health Insurance Plan, reported that it experienced a ransomware incident that affected the protected health information (PHI) of 868 individuals. The PHI involved included names, addresses, dates of birth, and Social Security numbers. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE strengthened its policies and procedures and implemented additional technical safeguards. Staff were retrained.
