OCR description
The covered entity (CE), Hope Group, reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 1,510 individuals. The PHI involved included names, addresses, dates of birth, health insurance information, diagnoses, medications prescribed, and other treatment information. In response to this incident, the CE provided complimentary credit monitoring and identity theft protections services and implemented additional administrative, technical, and security safeguards to better protect its PHI.
