OCR description
The covered entity (CE), Weddell Pediatric Dental Specialists, reported that an employee was the victim of an email phishing incident that affected the protected health information (PHI) of 5,356 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses/conditions, and other treatment and financial information. The CE notified HHS and the affected individuals. In response the breach the CE implemented additional administrative and technical safeguards.
