Oregon Eye Specialists, the covered entity (CE), reported that employees were the victims of an email phishing attack affecting the protected health information (PHI) of 42,612 individuals. The PHI involved included names, dates of birth, addresses, drivers’ license numbers, Social Security numbers, phone numbers, email addresses, passport numbers, state identification numbers, military identification numbers, tax identification numbers, financial information, diagnoses, prescription information, and health insurance and other treatment information. The CE notified HHS, affected individuals, and the media. Consequently, the CE ceased operations and is no longer in business.
