The covered entity (CE), Southern Orthopaedic Associates dba Orthopaedic Institute of Western Kentucky, reported that multiple employees were the victim of an email phishing attack that compromised the protected health information (PHI) of 106,910 individuals. The PHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, claims and financial information, diagnoses, lab results, medications prescribed, health insurance information, and other treatment information.
