OCR description
Alabama Department of Rehabilitative Services, the covered entity (CE), reported that it sold several audiology devices without removing the protected health information of 934 individuals. The PHI involved included the names, dates of birth, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. Following the breach, the CE sanctioned the responsible employee and implemented additional administrative safeguards to better protect its PHI.
