OCR description
The covered entity (CE), Fast Medical Center, reported that its business associate was the victim of a cybersecurity attack that affected the electronic protected health information (ePHI) of approximately 901 individuals. The ePHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, and medical conditions. The CE notified HHS, affected individuals, the media, and provided substitute notice.
