OCR description
The covered entity (CE), W. Emery Reynolds, M.D., reported that a laptop computer containing the protected health information (PHI) of 1,844 individuals was stolen from an employee of its business associate (BA). The PHI involved included names, Social Security numbers, dates of birth, and addresses. The CE notified HHS, affected individuals, the media, and provided substitute notice. While OCR was investigating the breach, the CE discontinued all services and closed its business; the case was therefore closed.
