OCR description
The covered entity (CE), Monongalia Health System, reported that multiple employees were the victims of an email phishing attack affecting the protected health information (PHI) of 398,164 individuals. The PHI involved included names, addresses, Social Security numbers, health insurance and claims information, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional technical safeguards and provided complimentary credit monitoring services to affected individuals.
