OCR description
The covered entity (CE), Chaddock, reported that an employee binder containing the protected health information (PHI) of 777 individuals was stolen. The PHI involved included names, addresses, and social security and drivers’ license information. The CE notified HHS and affected individuals. In its mitigation efforts, the CE took implemented additional administrative and technical safeguards to better protect PHI.
