Florida Digestive Health Specialists, LLP

The covered entity (CE), Florida Digestive Health Specialists, LLP, reported that multiple employees were the victims of an email phishing attack affecting the protected health information (PHI) of 212,509 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, financial and health insurance information, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional technical safeguards and retrained its employees on email security.
