OCR description
The business associate (BA), HOYA Optical Labs of America, reported that it experienced a ransomware attack affecting the protected health information (PHI) of 14,099 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, financial information, and prescription information. The BA notified HHS, affected individuals, and the media. In response to the breach, the BA strengthened its administrative and technical safeguards and provided complimentary credit monitoring services to affected individuals.
