OCR description
The covered entity (CE), A New Leaf, Inc., reported that it experienced a ransomware attack that compromised the protected health information (PHI) of approximately 10,438 individuals. The PHI involved included names, addresses, Social Security numbers, diagnoses, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. Following OCR’s investigation, the CE retrained its workforce members and implemented additional technical safeguards to better protect its PHI.
