OCR description
The covered entity (CE), Dr. Kochurani Puthumana, reported that its business associate (BA) was the victim of a ransomware attack that compromised the protected health information (PHI) of 3,805 individuals. The PHI involved included names, addresses, clinical information, diagnoses, and Social Security numbers. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE terminated its business relationship with the BA and retained another electronic medical records service provider.
