Sherrill House, the covered entity (CE), reported that multiple employees were the subjects of an email phishing scheme that affected the protected health information (PHI) of 5,006 individuals. The PHI involved included names, gender, addresses, phone numbers, dates of birth, drivers’ license and state identification numbers, Social Security numbers, financial and claims information, diagnoses, lab results, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards.
