The covered entity (CE), Vanderbilt University Medical Center, reported that an employee of its business associate (BA) was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 556 individuals. The ePHI involved included names, dates of birth, Social Security numbers, and treatment information. The BA notified HHS, affected individuals, and the media. The CE posted substitute notice on its website. The BA also provided complimentary credit monitoring and identity theft monitoring services and a phone number was provided for questions and concerns. In its mitigation efforts, the BA implemented additional technical safeguards to better protect sensitive data.
