OCR description
The covered entity (CE), Heart ‘n Home Hospice, reported that several employees were the victims of an email phishing scheme that affected the protected health information (PHI) of 4,769 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, and treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect its PHI.
