OCR description
Vantage Holding Company, the covered entity (CE), reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 1,762 individuals. The PHI involved included names, addresses, birthdates, diagnoses, lab results, and medications. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect PHI. Staff was retrained.
