OCR description
The covered entity (CE), Volunteers of America Southwest California, reported that an employee was the victim of an email phishing scheme that affected the protected health information (PHI) of 1,300 individuals. The PHI involved included names and vaccination records. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
