OCR description
The covered entity (CE), Walworth County, Wisconsin, reported that several employees were the victims of an email phishing attack that compromised the protected health information (PHI) of 1,212 individuals. The PHI involved included names, dates of birth, addresses and diagnoses. The CE notified HHS and the affected individuals. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. Staff were retrained on email security. OCR provided technical assistance to the CE regarding the HIPAA Rules.
