OCR description
The business associate (BA), Caring Communities, reported that its third-party cloud storage vendor was the victim of a ransomware attack that compromised the protected health information (PHI) of 1,659 individuals. The PHI involved included names, addresses, Social Security numbers, diagnoses, and other treatment information. The BA notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the BA provided complimentary credit monitoring services to affected individuals and terminated its business relationship with the third-party cloud storage vendor.
