OCR description
The covered entity (CE), University of Arkansas for Medical Sciences, reported that an employee emailed the protected health information (PHI) of 518 individuals to a personal email account. The PHI involved included names, dates of birth, medications, and health insurance information. The CE notified HHS, affected individuals, the media, local and federal law enforcement, and provided substitute notice on its website. OCR provided technical assistance to the CE.
