OCR description
The covered entity (CE), Oklahoma Health Care Authority, reported that its business associate (BA) discovered a configuration issue within a patient portal that compromised the protected health information (PHI) of 8,629 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, and account information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional technical safeguards to better protect its PHI. OCR provided the CE with technical assistance regarding the requirements of business associate agreements.
