EPIC Pharmacy Network, Inc.

The covered entity (CE), EPIC Pharmacy Network, reported that several employees were the victims of an email phishing scheme that affected the protected health information (PHI) of 28,776 individuals. The PHI involved included names, addresses, Social Security numbers, dates of birth, claims information, diagnoses, and medications prescribed. The CE notified HHS, affected individuals, the media, and provided substitute notice. Complimentary credit monitoring services were also provided. In its mitigation efforts, the CE implemented additional technical and security safeguards to better protect its PHI. In addition, the CE retrained its staff on email security.
