OCR description
The covered entity (CE), Family Fare, reported that its business associate (BA) experienced a hacking incident that affected the protected health information (PHI) of 3,892 individuals. The PHI involved included names, dates of birth, and prescription information. The BA notified affected individuals, the media, and provided substitute notice. The CE notified HHS.
