Cooper University Health Care

The covered entity (CE), Cooper University Health Care, reported that an employee was the subject of an email phishing scheme that compromised the protected health information (PHI) of 1039 individuals. The PHI involved included, names, dates of birth, claims information, medical record numbers, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. In addition, the CE retrained its workforce members on email security.
