Bible Fellowship Church Homes, Inc. dba Fellowship Community

The covered entity (CE), Bible Fellowship Church Homes dba Fellowship Community, reported that it was the victim of a hacking attack that compromised the protected health information (PHI) of 3,500 individuals. The PHI involved included names, addresses, dates of birth, claims and financial information, diagnoses, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional technical safeguards, revised its policies and procedures, retrained its workforce, and provided complimentary credit monitoring services to affected individuals.
