OCR description
The covered entity (CE), Memorial Village ER, reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 80,000 individuals. The PHI involved included names, Social Security numbers, dates of birth, addresses, phone numbers, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards. Staff were retrained to better protect its PHI.
