OCR description
The covered entity (CE), Ascension Michigan ACE, reported that an employee impermissibly accessed the protected health information (PHI) for 27,177 individuals. The PHI involved included names, addresses, dates of birth, social security and drivers’ license numbers, claims information, diagnoses, lab results, and medications. The CE notified HHS and the affected individuals, and published media notice. In response to the breach, the CE implemented additional administrative and technical safeguards. OCR provided technical assistance.
