Community Association of Progressive Dominicans, the covered entity (CE), reported that multiple employees experienced a phishing attack that affected the protected health information (PHI) of 656 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, medical record numbers, health insurance information, medications, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards. Staff were retrained on email security.
