OCR description
The covered entity (CE), Cooperman Barnabas Medical Center, reported that an employee of its business associate (BA) was the victim of an email phishing attack affecting the protected health information (PHI) of 538 individuals. The PHI involved included names, dates of birth, and claims and treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the BA implemented additional technical safeguards and provided training to its employees.
