OCR description
The covered entity (CE), CareOregon Advantage, reported that a workforce member sent an email containing the protected health information (PHI) of 10,467 individuals to the wrong recipient. The PHI involved included names, dates of birth, and health insurance information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach the CE retrained the workforce member on email security. OCR provided technical assistance to the CE.
