OCR description
The covered entity (CE), Bako Diagnostics, reported that it experienced a hacking incident that affected the protected health information (PHI) of 25,745 individuals. The PHI involved included names, dates of birth, addresses, telephone numbers, email addresses, drivers’ license and Social Security numbers, health insurance information, medical records numbers, and claims information. The CE notified HHS, affected individuals, the media, and provided substitute notice. The CE implemented additional administrative, technical, and security safeguards.
