OCR description
The covered entity (CE), The Art & Science of Dermatology, reported that a desktop server containing the electronic protected health information (ePHI) of 4,500 individuals was missing and may have been accessed by an unauthorized individual. The ePHI involved included names, dates of birth, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. Soon after the breach occurred, the CE notified OCR that they were no longer in business and the investigation was subsequently closed.
